Job Description:

To support the registered/licensed nurses by assuming responsibility for direct and indirect patient care in an effort to provide optimum patient outcomes.  He/she assists with activities of daily living, personal care, comfort measures, maintenance of a safe and clean environment, and certain tests/procedures done to a specific patient population under the direction of the registered/licensed nurse.

Job Accountabilities

Provides personal care to patients in a timely, accurate, and compassionate manner.  This includes hygienic care, activities of daily living, assistance with meals as needed, responding to requests by patients and other care needs as they arise.

Collects vital signs, intake and output, blood sugars, pain levels and other patient information as directed by the licensed nurse. Reports to the licensed nurse any changes in patient status, patient behavior, or unusual findings of data or incidents that might have occurred.

Documents accurately in the medical record the care that was delivered, activities completed, and other data that is collected for the patient.

Maintains clean and orderly patient rooms and unit.  Is responsible for keeping unit, nurse servers, and nutrition room adequately stocked with supplies.

Follows policies and procedures as defined by the institution.  Participates in unit improvement plan and process changes that enable us to provide care to the patient.

Education/Experience Level

High School graduate or equivalent required 

Current BLS required

Six months prior experience as a Nurse Aide in a hospital environment or completion of a Nurse Aide training program required (second year nursing student would qualify)

PCT certification or CNA license desirable

Basic computer skills

Knowledge and Skills ?

  • Must demonstrate competencies in: current caregiver practices (unit and age specific); data collection practices (vital signs, intake and output, etc); infection control guidelines including proper use of personal protective equipment and isolation classifications
  • Strong interpersonal skills to deliver direct and indirect patient care and interact with other members of the healthcare team
